Programs
We have multiple levels of care which would be customized to meet the individual needs:
- Residential program in their own home
- Supervised apartment program living
- Duplex living
- Rental home living
Programs Include
Program Management:
- Coordinate all medical and vocational appointments
- Implement customized multi-faceted program
- Written monthly reports
- Attend Team Conference - monthly, or as needed
- Implement and monitor budget techniques
- Monitor vocational - work placement
Room and Board:
- Pro-rate room and board charges between Insurance and client's ability to pay
- Apartment, Community Based - Furnished or unfurnished
- Food allowance (as needed)
Supervision:
- Structured supervision, daily phone calls, weekly visits
- 1:1 client to staff ratio twice per week
- Attend all medical appointments with client
- Monitor medication
Recreation:
- 2-5 Functional community activities weekly
- Leisure planning assistance / Reintegrate into community
Transportation:
- To/from all scheduled medical appointments within 15 mile radius (additional charges beyond 15 miles)
